We provide high-quality integrated medical care and a client-centered approach to substance use support which is grounded in harm reduction. These essential services are all designed to help individuals and to improve community well-being. We are committed to providing quality care, regardless of your ability to pay. Because we are a Federally Qualified Health Center (FQHC), our organization can provide more affordable care. Talk to our staff about our sliding scale fee structure, which allows you to access healthcare services for a reduced or free price. LEARN MORE Housing Works Community Healthcare is a Federal 330 Health Center Program grantee under 42 USC 254b and receives HHS funding. All health centers have Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. We provide equal access to quality health care to all persons regardless of age, sex, color, ethnicity, national origin, or the ability to pay or source of payment. There is a discounted/sliding fee schedule available based on family size and income. HWCHC has been awarded recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home program. This entity receives HRSA Health Center Program grant funding under 42 U.S.C. 8 254b and has been deemed a Public Health Service employee for purposes of certain liability protections, including Federal Tort Claims Act coverage, under 42 U.S.C. 6 233(a)-(n).
